Volunteer Interest Form

To ensure the safety and well-being of survivors and their families, individuals serving in direct service volunteer roles must meet the following requirements:

Be at least 18 years of age
Agree to and successfully complete a fingerprint-based background check
Commit to a minimum six-month volunteer term
Provide a copy of a valid driver’s license (if volunteer duties involve transportation)
Consent to and pass a motor vehicle report (if transporting program participants)
Provide proof of current automobile insurance coverage (if transporting program participants)
Participate in CPR and First Aid certification training provided by Circle of Hope
